CollabCare Wounds

The CollabCare wounds application allows a user to create a case that records the healing process of a wound.
The app can be used by a carer to capture both the wounds details and imagery over a period of time. Measurements and additional related information are calculated and presented back to the user within a Case page to support ease and accuracy of documentation.

During the life cycle of a wound, users are able to submit updates to track the healing trajectory. If a wound has become stagnant or worsening a carer is able to request a response from a clinical specialist to review and provide support. By using the CollabCare wounds application, an end-to-end solution that not only supports the patient but also the carer and clinician is made possible.
