Its very simple, you make your own list of things, maybe that pile of Books you're waiting to read or the students in your class. Then Pick 4 Me randomly selects one from the List for you.
When an item is picked, it is marked in the list, faded out, so that it won't be re-selected.
You can manually add these items back in - Just tap on them.
