CGM CARE MAP Mobile is an App, enabled by health care facilities, which allows you to:
- monitor the main vital parameters through automatic connection with medical devices
- collect information on symptoms and questionnaire responses
- receive notifications about planned activities
- share educational material to support patient empowerment
- communicate via chat and teleconsultations with health care personnel
The use of the App is bound by the activation by the healthcare facility that will take care of the visualization of the data sent by the patient, the processing and the intervention according to the telemonitoring service offered.
