Luchtbrug

With Luchtbrug we replace some of the hospital checks with online care.

Luchtbrug is an online tool with which we replace some of the outpatient checks with online care. Depending on how you are doing, you will have to come to the outpatient clinic less often. You regularly receive a short online questionnaire via Luchtbrug with which we follow how you are doing, this is also called online monitoring.

Via Luchtbrug you can also easily contact your care team and view your individual care plan. There you will find all information about medication, goals and an action plan in case of complaints.
