NYDocSubmit enables residents of New York State to submit required documents for SNAP, HEAP, Temporary Assistance, and Medicaid – avoiding another trip to the local social services district (“district”) office.
This App is available to residents of Albany, Allegany, Broome, Cattaraugus, Cayuga, Chautauqua, Chemung, Chenango, Clinton, Columbia, Cortland, Delaware, Dutchess, Erie, Essex, Franklin, Fulton, Genesee, Greene, Hamilton, Herkimer, Jefferson, Lewis, Livingston, Madison, Monroe, Montgomery, Niagara, Oneida, Onondaga, Ontario, Orleans, Oswego, Otsego, Putnam, Rensselaer, Rockland, Saratoga, Schoharie, Schuyler, Seneca, St. Lawrence, Steuben, Suffolk, Sullivan, Tioga, Tompkins, Ulster, Warren, Washington, Wayne, Westchester, Wyoming, and Yates Counties at this time. DO NOT use this App to submit an initial application for SNAP, HEAP, Temporary Assistance, or Medicaid; to submit a SNAP Interim Report, SNAP Change Report Form, or SNAP Periodic Report; or to submit a recertification application for SNAP, HEAP, or Temporary Assistance. However, you may use NYDocSubmit to submit a Medicaid recertification.
DO NOT submit sensitive information, such as HIV status or domestic violence information and/or addresses that must remain confidential to safeguard you or a household member. If you need to submit such information, or if the App is not available, provide the documents to your district in a manner OTHER THAN through this App, such as through the U.S. Postal Service, in person, kiosk (if available), or fax machine.
