The app can be used to:
• Order your blood pressure cuff
• Connect your account to Apple Health to pull health-related data from your personal devices right into the app
• Find educational resources
• Complete physician assigned tasks
• View blood pressure measurement results
• And more…
To use the Medtronic High Blood Pressure Care app, you must be enrolled by your participating healthcare practitioner. Upon enrollment, you will receive your login information via a confirmation email. If you have not received this email, please reach out to your healthcare provider to confirm your participation. Patients enrolled in the Medtronic High Blood Pressure Care program can use this app for the duration of their participation in the program.

The High Blood Pressure Care mobile app has requirements for your iPhone's operating system (iOS) version. The app cannot transfer data to your healthcare provider if those requirements are not met.

The requirements for your phone and iOS version may change over time. You may need to update, or replace, your iPhone and iOS to use the app to transfer data to your provider.

The High Blood Pressure Care mobile app is an optional tool to view and track your health data and blood pressure measurements. Talk with your health care provider for monitoring options.
This app is not a medical device.
This app is not for emergency health services.
