App Description
The Health Passport (HSE) helps share healthcare info for better care.
The Health Passport (HSE) is a communication tool to support people to express their needs in a healthcare setting.
This App includes sections on Critical Information, Personal Information, Communication, Decision Making, Medical History, Food and Drink, and Personal Care.
There is an option to email the passport if required.
The Health Passport (HSE) will support healthcare staff to get to know the needs of the passport holder, enabling them to consider reasonable adjustments before any assessment, examination or treatment of a person.
The App has an “Edit” and a “Read” function. The passport holder can enter information about themselves in the App, by selecting the Edit tab and choosing the section they wish to edit.
There is a “Prev/Next” section tab which allows you to go through the sections in the App.
Once the person has finished click “Save and exit”.
All information is saved to the passport holders personal device and can be viewed by opening the app and clicking on the relevant tab.
