It is mandatory for your patients to complete the general health assessment questionnaire and, for this reason, we have thought of streamlining the process for both you and your patients.
The questionnaire for assessing the general health of the patient comes in the version imposed by the Romanian College of Dentists, the minimum mandatory form, sufficient and in accordance with the provisions of Decision no. 3/2013 regarding the approval of the questionnaire for evaluating the general condition of the patient.
By filling in the information on the general health of the patient on the tablet, the patient's electronic file in iStoma will be automatically updated, and you will be able to consult it from any device.
For added security, we recommend that you print out the completed questionnaire to be originally signed by the patient.
