The JHWC Weight Management App connects you with your individually tailored wellness plan from JHWC. Receive daily meal planning, exercise guidance, lifestyle modification support and motivation.
As a JHWC patient, you will receive the specific content of your custom wellness plan in the app. Track your progress including calories, activities, body metrics, steps, water, and sleep in an interface shared with your provider. Communicate with JHWC in realtime through personal journals and direct private messages.
Intuitive Design
The app is designed specifically for JHWC patients and you can't use the app unless you're a patient already in their care. Use the app on your iPhone or iPad. JWHC uses an admin web-based portal where they can tailor the program to your needs, monitor your progress, and communicate with you directly.
