NCHC CareConnect is the official companion app for patients enrolled in care programs with Northern Counties Health Care (NCHC).

⚠️ HARDWARE REQUIREMENTS & IMPORTANT DISCLAIMERS ⚠️

No Smartphone Measurement: This app DOES NOT and CANNOT measure oxygen saturation (SpO2) or pulse rate using your smartphone camera or sensors.

External Device Required for Vitals: To use the vital sign tracking features within this app, you MUST use an external, compatible Bluetooth pulse oximeter device (provided by your NCHC care provider).

Companion Functionality: The app functions solely as a secure conduit to transmit data from this external hardware to your care team. All other care plan features within the app can be used independently without the external oximeter.

Enrollment Required: This app requires active enrollment in a care program with Northern Counties Health Care.
