PD Care is a digital device to support you between your doctor’s appointments. You may be enrolled in different types of monitoring programs, based on surveys or activity tracking.

PD Care activity tracking simplifies managing your treatment plan by helping you track your activities, your medications, and your pain levels. It also identifies patterns between your daily activities and pain symptoms, providing valuable insights for better pain management.

To use PD Care you must be a registered patient at an enrolled practice. An access code is given to you from your clinic. Download the app to your smartphone.

INTENDED PURPOSE: PD Care System is a digital health software solution designed to support individuals with long lasting pain and facilitate remote monitoring by healthcare professionals.

- PD Care is a self-management application designed for patients to track daily activities, respond to medical surveys, increase medication adherence, and receive personalized insights to identify behavioral adjustments for improved physical and mental well-being.  
The application promotes treatment adherence, empowers self-management, and enhances communication with healthcare providers.

PD Care System is an FDA registered Class I Medical Device.

PD Care System does not provide medical treatment, diagnostics, dose recommendations, or emergency alarms and does not replace proactive clinical care. It is intended as a supportive tool to assist patient self-management, medication adherence, and clinician oversight.

PD Care is not recommended for individuals who:

Have severe memory, thinking, or mental health challenges that affect their ability to use the app safely.

Are experiencing a serious medical emergency and need immediate care.

Have intense, uncontrolled pain that is not well-managed.

Cannot access or effectively use the required technology.

PD Care is designed for adult users. Those under 18 should only use it with guidance from a healthcare provider or caregiver.
