In this program Summit Health will provide you with devices that will sync directly with the Summit Health Connect app. By using the device, your health information will automatically be stored in the secure app and sent to your care provider without you having to input any data yourself. Your health will be monitored by your trusted provider in partnership with Summit Health Connect's Care Coaches to optimize your health.
