With POCKMED, your health is always organized. This pocket medical record digitally organizes and manages personal medical and health records in one place and helps to prepare for good doctor consultations. With the easy-to-use interface, one can keep together all personal medical and health records (PHR) conveniently in your pocket which can be easily accessed and shared. While each doctor or medical facility keeps a record of your care, it is important to have a complete record for your own use, so that you can share it with a new doctor, review at home to better understand your treatment, or manage your health insurance claims, taxes, and other legal matters. Once updated by the user, the POCKMED will become a compilation of all your medical information, test results, treatment reports, symptom diary or journal, pain mapping function, fitness tracker, emergency report, report for doctor, notes written by your health care team, etc. It is a perfect replacement for unreliable paper records or various digital platforms that that keep bits and pieces of your health and medical history. POCKMED team do not store any medical or personal data entered by the user in this app and we do not have access to anything you store using this application. App also has option to backup/restore data, create PDF reports and share options.

Most out of doctor visits: Medical professionals take important decisions related to a treatment plan or diagnosis based on the health and medical information given by the patient and the test reports. The Doctor Visit feature helps prepare information for effective consultation.

Symptom Diary or Journal: Note down details of any current & past symptoms. Entries in current symptoms are taken for report generation for doctor visit. Entries can be easily transferred to past symptom section & if required it can be brought back.

Pain Mapper & History: Pinpoint pain locations in your body on a human shape. Multiple pain points can be entered with pain intensity scale & color code together with a good description. Female/male,front/rear options are available. A pain history can also be easily created.

Doctor Notes & Prescriptions: Easily save and share prescriptions or any notes from doctor after each visit.

Report for Doctor Visit: Generate PDF report including part of the medical records entered by the user. Report can be shared easily.

In case of medical emergency (ICE): During medical emergencies, relatives can quickly and easily share critical medical data with medical professionals. The emergency card contains key details, and it can be printed as a PDF to share or print.

Medical Test Reports: Store any medical test report such as radiology, blood tests, pathology, etc. Details of the report together with an option to save a digital copy of the report in most of the common file formats that include image, document, audio and video. User can also give hyper link to records stored in cloud or internet.

Health Tracker: Chart and visualize various health and fitness parameters. A BMI calculator & body temperature converter from Fahrenheit to Celsius and back are included.

Links to external data: In some sections, user can save a hyper link to any medical records or data stored in internet or cloud.

Medications: Store details of current medications in this section.

Allergies & Reactions: If user is allergic, capture details in this section.

Medical conditions: Capture details of any existing medical conditions here.

Body implants, surgeries, procedures, hospitalizations, family history, mental health, unhealthy lifestyle are managed in medical data section.

Immunization or vaccination history can be easily managed in this section.
