Easy mobile access to MCT2D news, resources, and our Coverage Checker
The Michigan Collaborative for Type 2 Diabetes (MCT2D) was established in 2021 to accelerate a new way forward, through collaboration, innovation, and investment in quality, evidence based care for type 2 diabetes. MCT2D strives to remove barriers to guideline concordant care, support participating practices, and keep patients central to quality improvement work.
The MCT2D app provides much of the same information for our participating members and for members of the public as our website, mct2d.org, in a condensed format. The app provides a registration link to our next upcoming learning community event, news articles published by the team, a condensed version of our Coverage Checker tool, and our growing collection of resources for providers and patients based on our CGM, medication, and low carb diet initiatives. More details on each part of our app are below.
Coverage Checker
MCT2D collects and maintains a curated set of coverage data for CGM, GLP-1 RA & GIP, and SGLT2i medications for specific popular medical plans in the state of Michigan. Users can select a product type, plan type, and plans from the dropdown lists. After making their selections, the user is presented with a list of preferred and non-preferred options covered by their selected plan. If available, there are also links to formularies or other relevant documents.
Tools & Resources
MCT2D's curated selection of resources is adapted for easy access on the mobile screen. As with the resource library at mct2d.org/resource-library, the resources are filterable by various categories. For specific resources, mobile-specific versions of the tools have been designed to be more readable on a phone screen. Users can also bookmark their favorite tools for quick access later.
Optionally, users can share links to these resources via email or text by providing a phone number or email. We do not share this data with anyone.
